it does't take time for time to change

I always wondered why it takes only father's name in a form to make one's identity , why half of the forms never bothered about asking mother's name? Not that I didn't like using my father's name but all the while missed my mom's name. But it didn't take time much time to change when i had to write my mom's name as my guardian. Since he had left untimely.
